New Patient Registration

Register In Person

New patients are required to complete a paper registration form (PRF1) and a new patient questionnaire – these forms are available at our reception desk. 

Alternatively, you can print off the (PRF1) pdf form below. You will still need to complete a new patient questionnaire when handing in the (PRF1) form to our reception desk.

The completed registration forms should be returned to the reception desk any weekday when the practice is open. 

If you would like to register with Almondbury Surgery but live outside of our Practice Boundary please ask a member of our reception team for an “Out of Area – Choice of GP Practice Patient Information Leaflet”.
